I, as parent or guardian, hereby give permission for my child to play in the Victoria Red & White Boys Basketball League and acknowledge the fact that he is physically able to participate in league games. I, hereby authorize the director of the Victoria Red & White Boys Basketball League to act for me according to his best judgment in any emergency requiring medical attention, and acknowledge that I will be responsible for any cost (through family medical insurance or otherwise) incurred due to injury or sickness to my son. I hereby waive any claim that I might have against the Victoria Red & White Boys Basketball League and the Institution providing the facilities.

During the Red & White Boys Basketball League photographs or videos may occasionally be taken and included in publications and/or social media posts.  I, as the parent or guardian, permit the use of photographs and my child's name for publication and/or social media posts.
